We accept and will bill the following insurance carriers:

- PHCS (Private Healthcare Systems)

- United Healthcare
*Medicare members are responsible for the refraction fee at time of service. Members may also be billed for their yearly deductible.
Payment:
Patients are responsible for their co-pay at time of service.
Payment for all non-covered services will be collected at time of service.
We accept cash, check and all major credit cards.

That depends on how your visit is billed and what your insurance carrier will cover.
How is my visit billed?
How we will bill your insurance depends on the intent of your visit. Your visit will be billed as:
- Medical Visit
- A medical visit is indicated if you are having a medical eye problem or if you are referred for a medical reason.
- Routine Visit
- A routine visit is any exam that pertains to glasses and/or contact lenses.
What does my insurance cover?
Most insurances do not cover routine eye exams.
We advise you to call your insurance carrier in advance to verify your routine benefits. Please ask the following question(s):
- Do I have routine vision coverage? If yes, continue.
- How often do I receive the routine vision benefit?
- When was the last time the benefit was utilized?
- Do I have a co-pay or is there a specific dollar amount my insurance will cover?
- Does my insurance utilize a separate vendor for routine vision? (i.e. Vision Service Plan, Coordinated Vision, Cole Vision etc.)
- What are my benefits for glasses?
- If you wear or want contacts,
- Do I have contact lens examination coverage?
- Are the contact lenses themselves covered?
